Doctors Tell Women to Ditch Annual Pap Test

There may be no need to feel guilty about putting off your annual trip to the gynecologist anymore! New guidelines from the American College of Obstetricians and Gynecologists assert that healthy women should not get cervical cancer screening pap tests every year. Instead they encourage women to get the screening every two to three years. The medical group also recommends against getting a pap test until after the age of 21, even if an adolescent is sexually active.

The new recommendations against annual cervical cancer screenings are not intended to save you embarrassment or discomfort - they're meant to prevent unnecessary and possibly harmful treatments. For example, treatment of precancerous HPV symptoms that would otherwise go away on their own puts women at a higher risk of giving birth to a premature or underweight baby. Still, cervical cancer rates have dropped 50 percent since the 1970s mostly thanks to the pap test, so I hope the doctors aren't trying to fix something that isn't broken. But the authors of the guidelines explain that the same results can be accomplished with less-regular screenings.

I'll admit the idea that less screening can be better for your sexual health seems counterintuitive. Will you stick to your annual pap test or do you welcome a chance to get one less frequently?

US STD Rate Highest of Developed Countries - and Rising!

Let's talk about sex . . . or else! According to the Centers for Disease Control and Prevention, reluctance to talk about sex is keeping STD infection rates on the rise in the US, especially among teens. After practically being eradicated, syphilis is resurgent, and chlamydia and gonorrhea are also at "unacceptably high levels." Thanks to our prudishness, the US has the highest rate of STDs of any developed country in the world.

It's no secret that abstinence-only sex education is popular in America. Thankfully, after confronting the fact that abstinence-only education does not effectively reduce STD infection rates, some school districts are moving to abstinence-plus education, which reasonably stresses the importance of using protection if a teen is sexually active.

Of course, parents can also do something to reduce STD risks by providing a little sex education at home. When I asked a while back, more than half of TrèsSugar readers said their parents avoided having an awkward conversation about contraception. Maybe these new statistics will remind parents that reducing health risks are worth a few uncomfortable minutes.

Uh Oh! Counterfeit Condom Factory Discovered in China

Condoms might top the list of things that should never be counterfeited. Unfortunately, there may be as many as one million ineffective knock-off condoms on the market in China right now.

Four people were arrested this week at a factory allegedly producing and selling condoms that provide little to no protection against disease or pregnancy. What's worse? Authorities said they found "bare-chested" employees using vegetable oil to lubricate the condoms that they then packaged without any sterilization. Whoever came up with this fake factory sounds like they were fulfilling some sort of bizarre fantasy.

This story is scary enough to make someone outside of China paranoid. If you're freaked out, just make sure you never buy condoms labeled as "Jisson, Durex. Rough Rider, Six Sense and Love Card."

Condoms vs. the Pill: Which Do You Prefer?

When it comes to preferred methods of contraception among women, condoms have now caught up to the contraceptive pill in the UK. According to a new survey, condoms and the pill are equally popular among women, and those who said they preferred condoms cited its ability to protect against STDs. Of course I'm sure many cautious women use both methods, and with all the other contraception options out there, maybe you use neither! But if you had to choose between the condom and the pill, which do you prefer?

E-Health Records Raise Privacy Concerns About Sexual Health

In February, the US government mandated that doctors and healthcare providers create an Electronic Health Record (EHR) for every American by 2014. This new system, which healthcare providers and government officials can access when authorized, promises to make health care more efficient and less expensive. But there's a growing concern that the adoption of a central, online system could compromise patient privacy.

Some people worry that private information, such as someone's sexual health history, could get in the hands of the wrong people if health information becomes too centralized. One congressman, Rep. Patrick Kennedy, was recently asked specifically if information about STDs and abortion would be included in a person's record. He said that patients would be able to opt out of including such personal information in the electronic file.

Life is always easier when you swap out a paper-intensive system for searchable, sortable electronic databases. But when it comes to health records, there are risks to consider. Would you choose not to include sensitive information about your sexual health in your official e-file?

Huh? STDs Make Some Men Feel More Manly

I came across a Swedish study out earlier this week that is just too good to pass up. Get this: a significant number of young men in Sweden consider contracting an STD to be an affirmation of manhood, rather than a serious health issue.

None of the young women in the study (not surprisingly) considered an STD diagnosis as a sign of their "success with the boys." The study did find something both genders had in common: hesitation about using condoms. The young women feared upsetting their partner by asking him to use one, and the young men worried that taking out a condom would make them look as if they assumed they would get lucky that night.

While I don't think we should stigmatize people with STDs, it doesn't seem good for anyone to glorify preventable diseases. Do you find the study's results shocking or predictable?

Dr. Glickman's Answer:

YES!!!

There are two types of herpes virus: HSV-1 and HSV-2. Oral herpes is usually HSV-1 and it’s estimated that 80 percent of the population has been exposed to it. Genital herpes is usually HSV-2 and it’s estimated that 25 percent of women and 12 percent of men have it. However, genital herpes can be caused by either version of the virus, so going down on someone when you have a cold sore (another name for a herpes outbreak) can spread it. To make things a bit more complicated, only about 1/3 of the people who have genital herpes are aware of it - either they’ve never had an outbreak or the outbreak was very mild. Part of what makes herpes tricky is that it can be transmitted even when you don’t have a visible outbreak. The virus lives in your nervous system and every so often, it’ll come to the surface. It’s especially likely to happen when you’re sick or have been stressed for a while. There’s evidence that the virus starts shedding from the skin (and is therefore potentially contagious) before any visible symptoms. It’s a low risk, but it’s a possibility. To make matters worse, if the site of the shedding isn’t covered by a condom, it can be spread even when you’re practicing safer sex.

While herpes can be uncomfortable, it doesn’t represent nearly as much risk as other sexually transmissible infections (STIs) such as chlamydia, gonorrhea, syphilis, human papilloma virus (HPV), or HIV. Having said that, there’s evidence that HSV-2 makes HIV transmission more likely. See this article for more information about how that works.

It’s also worth mentioning that I’ve seen a lot of people get really upset about STIs when they don’t get nearly as upset about any other health issue. I’ve also seen a lot of people who have all sorts of judgment about STIs but think nothing of going to work and sharing their flu germs (or worse)! STIs can be unpleasant, annoying, or life-threatening, but they don’t mean anything about the person who has them any more than chicken pox or measles do. Negative attitudes about STIs are a holdover from before we invented antibiotics and when diseases such as syphilis were seen as the wages of sin. But now we know that they’re just germs and we don’t need to shame people for having them. (Not that I’m saying that you’re doing that, but I wanted to mention it.)

The best thing that we can all do is to get tested for STIs regularly, even if you’ve never had a symptom. Lots of people have STIs and don’t know it. Check your local Planned Parenthood or other agency for information, although I recommend going with anonymous testing in order to ensure your privacy.

And safer sex is always a good choice. It’s easy to do, with a little practice. Using a lubricant can make safer sex more enjoyable since latex tends to have a bit more friction than skin. Try a silicone-based brand if you’re prone to yeast infections. Water-based brands are a bit easier to clean up since they rinse away.

If you or your partner don’t like the way condoms feel, try a different brand. The Japanese-process condoms such as Maxx and Skinless Skin are more sheer than other types. Or you could give Skyn a whirl. It’s made from polyisoprene, rather than latex, so it’s very sheer and both people will get more sensation.

Given that you have oral herpes, you might want to talk with your boyfriend about using condoms for oral sex. If you go that route, change the condom before having intercourse. That will make sure that you don’t spread the herpes virus to your vulva or vagina. It’ll also reduce the risk of your condom breaking; teeth can cause small abrasions that weaken the condom.

Test Time! High School to Offer Free Pregnancy, STD Tests

In addition to math, science, and English exams, a New York high school is offering its students an even greater variety of tests: they will now have access to free and confidential pregnancy and STD tests.

Officials at Port Chester High School say they just want to offer students comprehensive health services. The school's clinic, run by Open Door, already offers immunizations, physicals, and vision and hearing tests. But some parents worry that the confidential nature of the STD and pregnancy tests will cut them out of their children's important health decisions. And the president of the school board, who opposes the plan, says it will turn the school into a quasi-maternity clinic.

Female Marine Recruits More Lax About Sex Than Civilians

Hoping to learn how to prevent unintended pregnancies from impacting combat readiness, the Marine Corps took a look at the sex lives of female Marines. The study revealed that female recruits had higher rates of the most common STDs (chlamydia and gonorrhea) and were less fond of using condoms than the general civilian population. The survey also revealed more lax attitudes toward mixing sex with alcohol among the new female Marines.

When you're in the armed forces, your sex life can impact the country's national security. Hoping to avoid losing Marines to unplanned pregnancy or illness, the Marine Corps includes sex ed in basic training. Each female recruit also has to have a 30-minute informal discussion about reproductive health with a senior drill instructor. That's got to be a little awkward - drop and give me birth control!

Early Testing Could Halt the Spread of HIV

It's estimated that about 250,000 Americans have HIV, but don't know it. Recently infected people could be the source of 10 to 50 percent of all new HIV transmissions.

A change in testing procedure might be needed to address the problem. According to public health officials, standard HIV tests do not identify an infection that has occurred in the last month, but a genetic test known as RNA can. Unfortunately, these early detection tests aren't widely adopted. If they were, lives could be saved.

This story is a reminder of how important it is to keep up on your sexual health. Do you make it a priority to get a routine HIV test?
